Search results allow you to drill down through the MeSH terms. Additionally, the results include concept terms and a feature called "Hot Topics", that will tell you which topics, journals, and authors are important to your search query. The result set is limited to 1000, but that's enough to give you a good idea of the potential of this application. Using the system, new surgeons could be trained, and experienced surgeons could gain perspective on their work, including the way that their actions could be made safer or more efficient. JHU Whiting School of Engineering professor of computer science and principal investigator for the project, Gregory D. Hager, explains, "Surgery is a skilled activity, and it has a structure that can be taught and acquired. We can think of that structure as the 'language of surgery'...we're borrowing techniques from speech recognition technology and applying them to motion recognition." Just as speech recognition breaks down words into their most elemental sounds, the team is breaking down surgical action to its most basic gestures, which software can represent mathematically, Hager says. To teach the robot the "language of surgery," the team is using data recorded by the da Vinci robotic surgery systems to construct mathematic models for specific procedures. Hager says that using the da Vinci data, their software has become able to go "from words to sentences," and they are progressing toward their goal of creating a "large vocabulary." The project is backed by a three-year National Science Foundation grant.

Full Article
Health Library Blog. According to their site Sermo is a place where specialties become communities and physicians ask and answer questions of each other in real time -- without any advertising or pharmaceutical promotion. The bottom line is that physicians can now access their colleagues' medical expertise the minute they need it, and working together, make decisions in a powerful new way."

Sermo's motto is: Know More. Know earlier. The site reports the following example of how Sermo has benefitted physicians.
" When three physicians independently reported that their patients on Lipitor were having nightmares, a follow-up survey was triggered on Sermo. The Sermo community was sent a "posting" asking if other physicians were hearing the same complaint from their patients. 33% of the 750 physicians surveyed said... read more

http://www.sermo.com/
Health Library Blog. He points out that many missed the author's statement about the Internet being particularly useful in the hands of a physician or other healthcare experts. "Clinicians have a better handle on which symptoms are unusual and for the technical terms for the symptoms," he says. The more precise the search terms, the more accurate the diagnosis. "Also, there's a lot of rubbish on the Web, and an expert can weed that out quickly."

The comments to the post are also interesting, indicating a tension between the fear many experts have of people depending exclusively on Google for information and those who have stories to tell of how Google helped them with a diagnosis. One post from an obviously frustrated health professional says "Nothing is worse then a kid coming to the ER at 4 am and a parent demanding some test or procedure becuase they read it in google. I actually have been told I'll be sued if I don't order a Cat Scan."

The tension will only grow as more people go online looking for health information. Like most things, there is no one single perfect solution to finding the best information, or in fact the "best evidence." I'm curious to know if there is in fact a solution, or potential solution in the technology itself, through the use of Web 2.0 tools that can help with more than just information retrieval, but with the discovery of information that you didn't know you were looking for in the first place . More to come.
Health Library Blog. I saw a bit of outrage on some of the library listservs and blogs, as well as a thoughtful post by a librarian.

The letters to BMJ are perhaps even more interesting than the article itself. Whatever you may have thought of the article or of the use of Google for a diagnostic tool, the dialog generated by the article is important.

One letter compares the search results in Google to those found in PubMed. Another letter mentions that perhaps the need for full-text searching or data mining is an important factor in using Google when bibliographic database searching isn't sufficient. I think part of what Google does is enable the *discovery* of information that may not be as discoverable in more traditional databases. Further studies are needed and I wonder what the results would be if the authors had used Google Scholar. More on this later. Take a look at the reactions to the article in BMJ.

Reactions
Health Library Blog. Very few check the source and date of the information they find.

Not really a surprise, but important information in a way. Consumers don't start looking for health information at any particular site. They don't go straight to the National Library of Medicine or to the numerous other excellent and reviewed sources.
So what does this mean? For one thing, it means that good information needs to be easy to find and once found needs to add value or provide opportunities for further discovery of pertinent information. Sure consumer education is important, but search engines, good search engines can filter by date etc. In fact Google does do some of this.

Pew Report

Online Health Search 2006: Most internet users start at a search engine when looking for health information online.
